2014 AFHE Convention Exhibitor Application

Thank you for your interest in being an exhibitor at AFHE's 31st Annual Arizona Home Education Convention Friday, July 11 and Saturday, July 12, 2014 at the Phoenix Convention Center.

Applying to be an exhibitor at the AFHE Convention is now a two-step process.  

STEP 1:  SUBMIT ONLINE APPLICATION
Complete and submit this online exhibitor application form.

STEP 2:  SUBMIT PAYMENT IN FULL
Once your application has been reviewed and approved, you will be provided with payment instructions.  You will have the option to pay by check (by mail) or by credit card (through an online payment gateway).  Your completed application AND your payment in full will reserve your booth space.  

AFHE reserves the right to decline an application without further explanation. If your application to be an exhibitor at the AFHE Convention is declined, we will notify you by email as soon as possible.

If approved as an exhibitor at the AFHE 2014 Convention, only company name and website will be published on the AFHE website and in the convention program.  The bulk of the information we are requesting is for AFHE purposes only.

NOTE:  Company names, contact names, and email addresses for registered exhibitors will be given to the service providers at the Convention Center in order for them to send service kits so that you may order any additional services such as electricity, Internet, extra tables, freight and materials handling, etc.


On-Site Representative Contact Information

If the primary person who will be on-site at the convention representing your company is the same as the company contact listed above, you can skip this section.

You will have the opportunity to provide names of additional representatives for name badges on page 3 of this application. If you do not know who those representatives will be yet, you may provide them by email to vendors@afhe.org by June 27, 2014 to have pre-printed badges prepared.
